That model is an attempt to explain how accidents/incidents can happen.
To prevent a predicted incident usually several methods (layers) are put in place.
Example: a machine:
Procedure for operation
Guards
Safety switch
Supervision
Soo, the incident only happens if:
The procedure is not followed.
Guards are removed.
Safety switch fails
Supervisor does not supervise.
Therefore all 4 holes align in the Swiss cheese.

If a random 17 year old can breach a security fence at an airport and get onto a plane with a gun then it is not hard to imagine what could of happened with someone who had a bit more skill. They could of easily taken the plane. Massive failure for the AFP and the labor government.
